An exchange between Dr Erica Schwartz and Senator Josh Hawley during her confirmation hearing is alarming Robert F Kennedy Jr’s new cooking show isn’t the only big news out of the public health world this week. On Wednesday, Dr Erica Schwartz was confirmed by the US Senate as director ‌of the Centers for Disease Control and Prevention (CDC), making her the first Black woman to hold the position. This is the first time in almost a year that the agency has had a permanent leader; the previous director, Susan Monarez, only lasted 29 days before she was ousted for butting heads with RFK about US vaccine policies. Translation: she was a little too keen on following accepted science for the Trump administration. So is Schwartz an RFK-approved vaccine skeptic who thinks you can cure cancer by praying to trees? Surprisingly: no. Schwartz, who served as deputy surgeon general during Trump’s first term, actually has government experience and a public health background. During her confirmation hearing in July, she walked a careful tightrope when asked about her stance towards vaccines. With their assault on science, Trumpists aren’t just harming America – they’re harming themselves | Jan-Werner Müller

The administration is subjugating science to political imperatives, a major blow to US soft power and technological progress The Trump administration is waging an unprecedented war on institutions devoted to research and higher learning. While plenty of countries have harmed universities by imposing neoliberal logics on academia – creating artificial markets in the name of spurring professors to be more productive – the Trumpist subjugation of all science to political imperatives has no equivalent in a democracy; if anything, it is reminiscent of Stalin’s Soviet Union. The assault now also targets foreign students and any kind of international collaboration – a major blow to US soft power. Why are Trumpists engaging in what can only be described as an act of wanton self-injury? Project 2025’s Russell Vought, the director of the office of management and budget, initiated the rule change that would make any federal grant subject to review by political appointees – including funds for science that so far have been allocated through judgment by first-rate fellow scientists. This measure would furnish Trumpists with a new weapon to threaten universities along the lines of: “Nice medical school you have there – pity if something were to happen to it because your administration had failed to come into alignment with the president’s priorities.” Jan-Werner Müller is a Guardian US columnist and a professor of politics at Princeton University..

As the harm caused by their products becomes more evident, manufacturers are resorting to denial and lawsuits to keep regulation at bay It is rare for people to drop dead holding the incriminating packaging, but there is no longer any serious doubt that consuming a diet of ultra-processed foods (UPFs) risks a catalogue of life-shortening illnesses from heart disease and stroke to diabetes and cancer. Because these diseases are degenerative over time, the food and drink industry has been able to argue for decades that its UPFs are fine in moderation. It would have us believe that there is no such thing as a “junk” individual food, only poor diets overall, and that its products therefore should not be subject to any restrictions. Just as with climate breakdown and smoking, the science on harm caused by foods high in fat, salt and sugar is now settled, but the industry continues to deny and distract to delay action. Xeno-monitoring the impact of Vector Control on trypanosome transmission in the Forecariah sleeping sickness focus (Guinea)

Author summary In January 2025, the World Health Organization validated the elimination of gambiense human African trypanosomiasis (gHAT) as a public health problem in Guinea. This goal was achieved through a combination of medical and entomological activities over one decade. This strategy was successively implemented in the three gHAT active foci, first in Boffa in 2012, then in Dubreka in 2016 and in Forecariah in 2018. Here, we present the evaluation results of the first three years (2018–2021) of vector control (VC) in the Forecariah focus. VC consisted in the annual deployment of over 5,000 blue insecticide-impregnated tiny targets, mainly in mangrove areas, which are at high risk of Trypanosoma brucei gambiense transmission through tsetse fly bites. Only 18 months after the initial deployment, the apparent density of tsetse flies (ADT) had decreased by 92%. This ADT drop was associated with a significant reduction of the trypanosome infection rates (from 11.5% to 1.2%) in tsetse midguts over the three years of VC. Prior to VC implementation, at least one infected fly was found in 18 of the 31 sentinel traps, whereas only two were positive after three years of monitoring. A pan-trypanosome PCR, used as a molecular xenomonitoring tool, revealed the predominant presence of T. brucei sl., while T. vivax and T. congolense were detected in few tsetse midguts. During this period, HAT prevalence decreased by 76%, highlighting the effectiveness of this combined strategy.

Development of a deep learning based framework for classification of Indian venomous snakes integrated with explainable artificial intelligence for primary and emergency care providers

Author summary Snakebite is a major public health concern that disproportionally affects the rural population. Delays in identifying whether a snake is venomous often lead to delayed treatment, unnecessary use of antivenom, or inappropriate referrals. In many rural settings, access to expert snake identification is limited. To address this gap, authors have developed an artificial intelligence (AI) based image classification system that distinguishes snakes into two clinically relevant categories: venomous or non-venomous. Unlike many previous studies that focused on ideal, high-quality wildlife images, our model was trained using real-world photographs captured in emergency situations, including images taken by patients and field responders under variable lighting and background conditions. This approach improves the model’s relevance to practical healthcare settings. The system achieved high accuracy and was further strengthened by visual interpretability tools and expert verification to ensure reliability. By combining AI-assisted classification with human oversight, this work provides a scalable decision-support tool that may improve early triage, rational antivenom use, and surveillance in snakebite-endemic regions

Dengue hospitalizations in Brazil: Forecasting with climatic and physicians’ digital search data under real-world reporting delays

Author summary Dengue is a major public health challenge in Brazil, where large outbreaks place sudden pressure on health services. Although climate conditions influence dengue transmission, public health responses often rely on hospitalization data that become available only weeks or months after patients are admitted, limiting the ability to act early. In this study, we explored whether real-time digital information generated by physicians could help overcome this delay. We combined weather data with anonymized records of physicians’ searches for dengue-related information within a widely used clinical decision-support platform in Brazil. We then tested whether these digital signals could improve short-term forecasts of dengue hospitalizations across different regions of the country, especially when official hospital data were delayed. We found that when hospitalization data were available without delays, simpler models based on hospitalization history and climate patterns performed well. However, under realistic reporting delays, models that incorporated physicians’ search behavior produced more accurate forecasts across most regions. These findings show that digital clinical behavior can provide early insight into rising disease activity and support more timely public health responses.

Conversational, Longitudinal, Ecological Assessment (CLEA): Exploring a new AI-driven method for qualitative data collection in a behavioural health context

Author summary Developing effective interventions for health behaviours such as healthy eating and physical activity requires methods that can capture the complex, individual factors shaping people’s everyday experiences, including stress and motivation. Because such factors often fluctuate over time, longitudinal approaches are needed to understand how experiences and behaviours unfold in real-world contexts. For such methods to be effective, they must also be acceptable, engaging, and accessible; particularly for underserved or disadvantaged populations known to experience lower health and digital literacy. In this study, we introduce conversational longitudinal ecological assessment (CLEA), a digital health method that uses conversational AI technology to collect ecologically valid qualitative data over time through an accessible instant-messaging platform. We demonstrate the feasibility, acceptability, and utility of CLEA through a real-world deployment investigating the experiences of individuals accessing a community-based weight management programme, being delivered in an area of deprivation. To support other health researchers, we position CLEA in relation to existing longitudinal qualitative methods and highlight the key design considerations that shape engagement, data quality, and participant experience.

Feasibility study of an AI-powered mobile app to support cutaneous leishmaniasis diagnosis in the Brazilian Amazon

Author summary Cutaneous leishmaniasis is a major public health challenge in the Brazilian Amazon. Delays in diagnosis increase illness, stigma, and costs, especially in remote communities with limited connectivity. Community health workers and primary care teams are often the first to see suspicious skin lesions, but they lack point-of-care tools to support early triage. We developed and evaluated the feasibility of a mobile application that works entirely offline and uses artificial intelligence to assist with early triage from smartphone photographs of skin lesions. The model was trained on clinical images collected by partner institutions in the Amazon region and integrated into an app optimized for low-resource settings. In a multicenter assessment that included both more standardized (“ideal-world”) images and routine field conditions, the tool identified most cutaneous leishmaniasis cases, returned a result in under a minute, and was well accepted by users. Specificity was moderate, which is appropriate for a triage tool designed to prompt timely referral rather than to replace clinical and laboratory diagnosis. This feasibility study shows that an offline, AI-assisted approach can help accelerate referral and access to care in underserved regions. Next steps include larger, prospective clinical validation, usability refinements based on field feedback, and regulatory evaluation aligned with Brazilian requirements (ANVISA). The same strategy could be adapted to other dermatologic conditions and neglected tropical diseases in similar settings.

World Health Assembly endorses resolution on precision medicine

At the Seventy-ninth World Health Assembly, Member States endorsed a new resolution on Precision medicine: a path towards targeted, personalized and equitable care, taking an important step towards advancing precision medicine as part of efforts to strengthen equitable, effective and sustainable health systems. Precision medicine refers to the use of clinical, molecular, genomic and other health data to inform prevention, diagnosis and treatment, taking into account variability while applying appropriate ethical and legal safeguards. When integrated responsibly into health systems, precision approaches can improve outcomes across different disease areas and across the life course – from cancer and rare diseases to infectious diseases, maternal, newborn and child health, mental health and noncommunicable diseases. The resolution reflects rapid advances in genomics, diagnostics, data science and digital health, alongside widening global inequities in access to these innovations. Many populations – particularly in low- and middle-income countries – remain underrepresented in data and research, limiting the public health impact of precision medicine and risking the widening of health disparities if action is not taken now. "Precision medicine is not an aspirational idea for health delivery in the future – it is already transforming lives across the world. But its benefits remain out of reach for too many and the research driving progress is concentrated in high income countries. This WHA resolution gives global health leaders, ministries of health, and WHO a clear mandate: to act. From infant screening to setting up genomic databases WHO will work with countries to bring about the power of these innovations." In fact, you could say it is the world's predominant healthcare system, either by primary access or preferred choice."However, despite the huge need and demand, Dr Kuruvilla noted that less than one per cent of global health research funding goes to traditional medicine.In a panel discussion that followed, investors advocated for blended finance models combining venture capital, philanthropic grants and government support to help navigate costly clinical validations and fragmented regulatory hurdles. Risk mitigation through broad portfolio diversification across multiple countries was also highlighted to attract private capital.Opportunities and priorities for AI and TMThe rapidly expanding influence of AI in reshaping global health, including TM, was highlighted frequently during WHA79, including during an official side event on 20 May, led by the Government of India together with Thailand, Nepal, Sri Lanka, and others. The course responds directly to requests from WHO Member States to strengthen capacity for clinical trials (as articulated in WHA75.8) and improve research quality, coordination and preparedness for public health emergencies.“Well-designed and well-conducted clinical trials are fundamental to protecting participants, producing trustworthy evidence and strengthening confidence in science,” said Dr Sylvie Briand, WHO Chief Scientist. “This course equips a wide range of actors with a shared understanding of good practices, helping to translate global guidance into better trials, better evidence and ultimately better health outcomes.”Clinical trials can involve anyone – healthy individuals or people living with a disease or other health condition. They test a wide range of health interventions, not just medicines. This includes vaccines, medical devices, surgical procedures, diagnostics, nutrition, physical therapy, psychological support, digital health tools, traditional or herbal remedies and process-of-care changes.Recent breakthroughs in global health underscore the critical role of high-quality clinical trials. These include long-acting HIV prevention injectables; innovative cancer therapies showing promising improvements in survival; advances in malaria prevention and treatment and new diagnostic tools to help end TB. “At a time when the global health agenda, and even science itself, is under attack, the importance of clinical trials cannot be overstated,” noted Dr Meg Doherty, Director of WHO’s Science for Health Department. “WHO remains committed to supporting the global research community to deliver life-saving advances in health, particularly in developing countries where the need is greatest. This new course forms part of our renewed focus on strengthening equitable research and ethics ecosystems.”Grounded in WHO’s 2024 guidance, the course is structured around five universally applicable scientific and ethical principles. Across nine interactive modules, taking approximately 4.5 hours to complete, learners explore how these principles can be applied in different settings, disease areas and health systems. The course emphasizes real‑world decision‑making, addressing common challenges in trial design, conduct and oversight, including participant protection, ethical review, community engagement, operational feasibility and relevance to public health needs. “The course provides a clear and highly practical foundation for ethical, high‑quality clinical research,” enthused Dr Rajeshree Sanyal, Senior Project Manager at The George Institute for Global Health in India, and an early course participant. “The real‑world case studies and interactive course format makes the learning process both engaging and easy to follow.”While currently available in English, other language versions of the course are planned to be rolled out in coming years. “The course successfully bridges the gap between ethical theory and operational reality, giving me the professional edge to ensure our trials are both robust and participant-centred,” explains Mithun Mohan George, Project Manager at Sunnybrook Research Institute in Canada. “I have already started implementing key lessons in my monitoring plan documents and would definitely recommend this course to all those who are looking to make clinical research practices better.”By making this course freely available online, WHO aims to reduce barriers to high‑quality clinical trials education and support countries in building sustainable research capacity aligned with national and global health priorities.
